HOW DOES A HAIR TRANSPLANT WORK?

Let’s go back to the beginning. In Japan in 1939 and 1942, Okuda and Tamura found that hair transplanted from the back of the head to replicate pubic hair loss would survive and grow. Due to public bathing rituals in Japan and an illness among young Asian women suffering from pubic hair loss, this type of transplant proved to be an important step in knowing that hair transplanted from one area of the body to another would thrive and survive. However, it wasn’t until the 1950’s when the famous New York dermatologist Norman Orentreich knew that hair moving from the back of the head to the front of the head where there is baldness would not be lost over time like the original hair was there. He called this phenomenon “donor dominance,” meaning that the hairs that move from the back of the head to an area of genetic susceptibility to hair loss retain the characteristics of the donor hair and are not lost over time. This was the brilliant breakthrough we needed to know that the results would persist despite transplantation into an area predisposed to hair loss.

If you then wonder why hairs in the back of the head are not prone to hair loss? Well, only God knows. However, it is the case. Think of the baldest man you know (who hasn’t shaved the hair on the back of his head). He’s got a strand of hair back there. Even the baldest man has a lagging hairy horseshoe in the back of his head. The only trick then in doing a hair transplant is knowing which area is “safe” for the transplant, meaning which area will not be lost over time as the person ages. This is one of the main reasons why transplanting a person in their 20s can be problematic. We just don’t know how much hair on the back of the head will not fall out over time. Also, we may simply run out of donor hair to transplant the front of the head and get a natural result as more hair (that has not been transplanted) falls out with age.

This verdict is really one of the main characteristics that separates an experienced hair transplant surgeon from a novice. Knowing who to operate on (that is, who is safe and who is not) is a prerequisite for a safe hair transplant. Using the laws of supply and demand, someone who has enormous donor hair density, ie there are many hair follicles per square centimeter in the donor area, can in many cases naturally and impressively conceal an enormous amount of baldness. A surgeon’s wise use of grafts in a good pattern distribution with good angulation helps ensure the result is both natural and dense, considering the degree of hair loss and the useable donor hair supply of a particular individual.

The other question that is often asked is, “Will the transplanted hair be the same as the other hairs I have there that have not been transplanted?” Am I going to cut it like my other hair?” The answer is a resounding yes. I further explain that in a hair transplant, hair is simply moved from one side of the head to the other, like taking a flower from one pot and placing it in another. It will grow in its new environment just as it did in its previous one.
